seva_session — set by this site to keep the appointment form secure (it holds the form's anti-forgery token). It is deleted when you close your browser and holds nothing about you.
Google reCAPTCHA — sets cookies on google.com to tell people and automated programs apart when you use the appointment form.
Analytics

Third-party features
The chat window (LeadConnector), embedded maps (Google Maps) and patient videos (YouTube, in privacy-enhanced mode) may set their own cookies when they load or when you use them.
